My 404.html is in /htdocs so it should be OK. After some hours it started to work without any modification... but I think I will follow your suggestion, it is more flexible and manageable.

It was not a problem related to browser cache or history, I checked also with different devices. Yesterday evening, after about 6 hours since the problem born, it disappeared without any apparent reason... I really don't understand why.
